What To Bring

  1. New Patient Forms – Please download and print out the forms below and ensure they are completed in full prior to your appointment. Our front office staff will collect them when you arrive for your appointment. It is very important to complete the section regarding any medications, their dosages and frequencies, in order to avoid adverse drug reactions.

  2. Previous Medical Records and/or X-rays – Please gather all x-rays, CT scans, MRI images/reports, office notes, operative reports and any other records from any physician who has seen you in the past related to your current orthopaedic condition. The more information you provide, the better we will be able to help you and less likely it will be that we would have to repeat any tests or treatments you have already tried.
  3. Your insurance card, photo identification and referral (if applicable)
  4. Comfortable, loose fitting clothing – For many orthopaedic issues, you may need to wear a gown during your evaluation. Please wear clothing that is easy to change.
  5. Consider asking a friend or family member to accompany you. If you need a translator, please ask another adult to help you during your appointment.
